Maasin City QR Code Registration

Maasin City QR Code is Maasin LGU's own and unique contract tracing system. Individuals may register a QR Code through the ARK Maasin web portal.

(Updated 09/11/2021) Since September 2020, Maasin LGU has implemented its own COVID-19 contact tracing system through the ARK portal, which provides a unique and distinctive quick response (QR) code for residents and non-residents of the city.

Maasin QR Code can be used alongside Southern Leyte’s own quick response (QR) code system called ACTS. An individual who will enter the city must present said QR codes. If you are a traveler, it must be presented in every port area or terminal of the city for contact tracing purposes if there is a risk of COVID-19 infection.

Residents and non-residents of Maasin City must have a QR code to enter establishments such as government offices, restaurants, malls, and shops. You may check this article to know how to get a Maasin QR Code.

What is Maasin QR Code?

Maasin QR Code is the COVID-19 contact tracing system by the local government unit (LGU) of Maasin City. It is used by residents and non-residents who want to enter facilities and establishments within the jurisdiction of the City.

Suppose an individual will be traveling or pass through Maasin City. In that case, they need to have a QR Code, especially for Authorized Persons Outside Residence (APOR), travelers (non-APOR), Returning Overseas Filipinos (ROFs), and Returning Residents (RRs).

As of July 5, 2021, Maasin City is placed under General Community Quarantine (GCQ) status, which means health, security, and safety measures are strictly enforced in the city.


Where to download or generate a Maasin QR Code?

To generate a Maasin QR code, you must go to http://arksouthernleyte.com/maasin/. From there, you may register for an account to get your own unique and distinctive, quick response (QR) code for Maasin.

You may also need to register at Southern Leyte’s ACTS portal, https://trace.southernleyte.org.ph/signup/, if you will be going to other LGUs in Southern Leyte aside from Maasin.

What are the requirements to get a Maasin QR Code?

An individual needs to have internet access to proceed with their Maasin QR Code registration. They must have a valid mobile number since this will be used for contact tracing purposes if there’s a risk of COVID-19 infection.


How do I get a QR Code for Maasin City?

To get a QR Code for Maasin City, you must register through ARK Maasin City at this web address: https://arksouthernleyte.com/maasin/. The link is accessible through desktop and mobile devices.
Upon going to the website, you must click on the “REGISTER” button to proceed with your Maasin QR Code registration.

You may also follow the easy and simple steps below:

1. Go to https://arksouthernleyte.com/maasin

Once you're able to go to the website of ARK Maasin's web portal, you may select "Register" if you're an individual who needs a QR Code or "Download Scanner" if your establishment needs a QR Code Scanner (for businesses in Maasin only).

To start your registration for a Maasin QR Code, you must go to their website. It can be accessed through any browser, but it is recommended to use the Chrome browser for Android devices and the Safari browser for iOS devices.


2. Click on the “Register” button 

Now click on the "Register" button to proceed with your Maasin QR Code Registration.

To continue your Maasin QR Code registration, you must click on the "register" button. Ensure to have a valid and active mobile number since this will be used for contact tracing purposes.


3. Fill out the Maasin QR Code registration form

After clicking on the register button, you must fill out a registration form with your complete information.

Once you are done with the steps above (Step 1 to 2), you may fill out the registration form and provide the following information: full name, complete address, date of birth, and active mobile number.


4. Agree with terms and click on “register”

Once you fill out the registration form, you must agree to Maasin QR Code's terms and conditions. Afterward, click on the register button.

After you complete filling out the registration form for Maasin QR Code, you must tick the box for LSI/ROF if you are a locally stranded individual or a returning overseas Filipino. If you're not an LSI/ROF, leave it and then agree with the terms and click on the "Register" button.


5. Receive your Maasin QR Code instantly

Your Maasin QR Code will be generated instantly. You must save a copy of it and print it. It is advisable also to laminate it.

Once you are done with your registration, the registration portal will immediately generate a QR code for you. Make sure that you will save it since you will need the assistance of LGU Maasin if you cannot keep a copy of your Maasin QR code.


Where is Maasin QR Code used?

Individuals who will be going to Maasin City are required to have a Maasin QR Code.  Establishments, government, and private offices within the jurisdiction of Maasin City are required to ask individuals for their QR codes upon entering their premises.

What are the travel requirements for individuals going to Maasin?

Under the National-IATF Resolution No. 124-B, Provincial Executive Order No. 37 of Southern Leyte and Maasin City LGU's Executive Order No. 31, Series of 2021, the following are the guidelines for all APOR and NON-APOR (travelers), including the Returning Residents (RRs) and Returning Overseas Filipinos (ROFs).

Note: Those who are considered fully vaccinated individuals are no longer be required to present negative RT-PCR test results and no additional quarantine will be imposed.

These guidelines are effective as of  July 1, 2021. (Note: Aside from the Maasin QR Code, a returning resident must have a Letter of Acceptance (LOA) from Maasin LGU if they want to travel to the said city. You may check the steps here.)


Returning Residents (RRs) from Luzon, Mindanao, or the Visayas

  1. Endorsement Letter of the Barangay with Final Approval from the City Mayor of the concerned LGU of final destination for the issuance of the Letter of Acceptance (LOA);
  2. Presentation and scanning of Maasin City QR Code/ACTS (Southern Leyte );
  3. Thermal Scanning and with a body temperature of not higher than 37.5°C; and
  4. Presentation of Valid Government Issued ID.

*Home quarantine is allowed provided a certification duly signed by the CHO or their authorized representative that the home quarantine area meets the required standard.


Authorized Persons Outside Residence (APOR)

Here are the guidelines for all persons working in the government/private agencies and institutions (national and attached agencies or local):

A. Those working in government/private agencies (national and attached agencies or local) outside Southern Leyte's Province who have business transactions within the province.

  1. Presentation and scanning of Maasin City QR Code/ACTS (Southern Leyte );
  2. Thermal Scanning and with a body temperature of not higher than 37 .5°C;
  3. Presentation of Valid Government Issued ID; and
  4. Travel order and Itinerary.


B. Those residents of Southern Leyte and whose work station is outside Southern Leyte, and those whose residence is outside Southern Leyte and whose work station are within Southern Leyte.

  1. Presentation and scanning of Maasin City QR Code/ACTS (Southern Leyte );
  2. Thermal Scanning and with a body temperature of not higher than 37 .5°C;
  3. Presentation of Valid Government Issued ID; and
  4. Certificate of Employment.


C. All people (ages 15-65 years old) and fully vaccinated individuals, including senior citizens, whose purpose of travel is to access basic/essential/health needs or for business purposes who travel in and out of Southern Leyte.

  1. Presentation and scanning of Maasin City QR Code/ACTS (Southern Leyte );
  2. Thermal Scanning and with a body temperature of not higher than 37 .5°C;
  3. Presentation of Valid Government Issued ID; and
  4. Medical Certificate signed by BHERTs (place of origin) with a validity of 5 days upon issuance of Vaccination Card for fully vaccinated individuals or Bureau of Quarantine (BOQ) Certification, whichever is applicable. 

D. For APOR passing through Southern Leyte going to Mindanao or other areas outside of Southern Leyte.

  1. Presentation and scanning of Maasin City QR Code/ACTS (Southern Leyte);
  2. Thermal Scanning and with a body temperature of not higher than 37 .5°C; and 
  3. Presentation of Valid Government Issued ID.


E. Men in uniform, in the military, or the policemen upon arrival in the province.

  1. Presentation and scanning of Maasin City QR Code/ACTS (Southern Leyte);
  2. Thermal Scanning and with a body temperature of not higher than 37.5°C;
  3. Presentation of Valid Government Issued ID; and
  4. Travel order and Itinerary / Certificate of Employment; and
  5. Strictly observe home quarantine for 7 days.


Non-essentials Travelers (Non-APOR)

Travelers are referred to previously as non-APOR. They travel for leisure, recreation, pilgrimage, or tourism purposes either coming from Luzon, Mindanao, or the Visayas  (inclusive of the Provinces of Leyte, Biliran, Samar, Eastern Samar, and Northern Samar, all of Region VIII).

  1. Presentation of Saliva/RT-PCR Test with Negative Result (Test must be taken 5 days before travel) or Vaccination Card for fully vaccinated individuals or Bureau of Quarantine (BOQ) Certification, whichever is applicable;
  2. Presentation and scanning of Maasin City QR Code/ACTS (Southern Leyte);
  3. Thermal Scanning and with a body temperature of not higher than 37.5°C; and
  4. Presentation of Valid Government Issued ID.

Local tourists (Southern Leytenos who travel within Southern Leyte) are required to present to the Tourism Office or to the deputized personnel/barangay officials of their destination, along with Barangay Health Declaration Form or Vaccination Card for fully vaccinated individuals duly signed by the BHERTs, valid ID and QR Code of the province. 


Returning Overseas Filipinos (ROFs)

  1. Bureau of Quarantine (BOQ) Certificate with attached Negative RT-PCR Test Result; and
  2. Vaccination Card.

Per IATF Resolution No. 123-C, Series of 2021 dated June 28, 2021, ROFs from "Green" Countries who underwent the required 7-day facility-based quarantine upon arrival in the Philippines will no longer be required to undergo quarantine upon entry to Southern Leyte. However, they are advised to stay at home and be closely monitored for five (5) days for symptoms monitoring.

How to get a Letter of Acceptance (LOA) from Maasin LGU?

This is the latest guide on how to get a Letter of Acceptance (LOA) if you are a returning resident of Maasin City. Just follow the steps below:

  1. Obtain a Certification from the Barangay of destination indicating that you are a resident of the said barangay.
  2. Send your Brgy. Certification to sppaleracasio@gmail.com, including the following details: name, address of origin, travel date, mode of transportation, and contact number.
