Step 2 – Arrive at your scheduled appointment date.

On the day of your appointment, arrive at your preferred PSA office at the time and date specified. For validation, show the CRS Appointment Slip you received from the Online Appointment System to the authorized personnel.

Step 3 – Complete the required form for your birth certificate application.
