On the following page, enter your complete name, email address (for OTP), and mobile number. After completing the form, check the box indicating that you consent to the collection of your data and then click the "Next" button to proceed.

Step 4 - Enter the OTP for verification and select the certificate to be processed.


After submitting the form, you will receive an email with a 6-digit one-time password (OTP) in your registered email address. Enter the OTP to verify your application, and then on the next page, select the type of civil registry document you wish to process.

Step 5 - Choose a date for your appointment and print the confirmation slip.


Following that, you must select an appointment date from the available slots. After selecting the time and date of your appointment, click the "Next" button to review the details of your schedule. Click the "Confirm" button after reviewing the details of your appointment schedule.

You will receive an appointment slip similar to the one shown below. Additionally, it will be sent to your registered email address, from which you can print the file.
