Samal City QR Code Registration

If you are a resident or non-resident staying in the Garden Island City of Samal, make sure to get your own QR Code. Know the steps here on how to register for a QR Code.

(UPDATED) Since July 2, 2021, all individuals (either residents or non-residents) in Samal Island must present their quick response (QR) codes upon entering establishments in the island city per Executive Order No. 301 Series of 2021.

The Samal QR Code is used for contact tracing purposes wherein all private, and public establishments are required to adopt and use the QR code system in their respective places per the city’s policies and guidelines on using QR Codes.

Individuals including returning residents, visitors with official business, authorized persons outside residence (APORs), and returning overseas Filipinos (ROFs) without QR codes are not allowed to enter the Island Garden City of Samal. What is Samal QR Code?

Samal QR Code is the official COVID-19 contact tracing system of the island garden city located in Davao del Norte. Although a component city of Davao del Norte (DavNor) Province, LGU Samal requires individuals to use the said QR code system as the primary contact tracing system and not DavNor's COVID-19 QR app.

Individuals, especially residents and non-residents working in Samal, can get a QR Code through the "ITRAC SAMAL" web portal. ITRAC means I-nformation of T-ourist and R-esidents A-gainst C-OVID-19 system. Individuals are also encouraged to get a DavNor QR Code to avoid delays and inconvenience while traveling (if a non-resident) to Samal City.

Where can I download or generate a Samal QR Code?

To download or generate a Samal QR Code, you must go to https://itracsamal.com/qr_codes and click on the "REGISTER FOR A QR CODE ID" button. All residents or non-residents must register a Samal QR Code by filling out the Samal QR Code registration form.

There are two types of registration for getting a QR Code in Samal: (1) Samal resident and (2) non-residents. If you are a resident of Samal, mark the checkbox beside "SAMAL RESIDENT," and if you are a non-resident, mark the checkbox beside the option "NON RESIDENT."

What are the requirements to get a Samal QR Code?

Residents and non-residents who want to get a Samal QR Code must have a stable internet connection and capable mobile or desktop devices to access the "ITRAC SAMAL" web portal. Individuals must also provide a digital or scanned copy of the following for each type of registration and entry type:

For residents working in Samal or outside the City:

  • Employment Certificate or Company ID; and
  • Barangay Certificate with Government Issued ID Indicating Residence in Samal.

For self-employed Samal residents:

  • Barangay Certificate; and
  • Government Issued ID Indicating Residence.

For non-residents working in Samal:

  • Employment Certificate or Company ID Indicating Address in Samal;
  • Government Issued ID Indicating Residence; and
  • Health Certificate Indicating Not a PUI or PUM (Issued by the City or Municipal Health Officer of the Place of Origin).

For business owners who are non-residents:

  • Business Permit;
  • Government Issued ID Indicating Residence; and
  • Health Certificate Indicating Not a PUI or PUM (Issued by the City or Municipal Health Officer of the Place of Origin).

For resort owners who are non-residents:

  • Land Title;
  • Building Permit;
  • Government Issued ID Indicating Residence;
  • Tax Declaration with 2019 Tax Clearance;
  • Business Permit; and
  • Health Certificate Indicating Not a PUI or PUM (Issued by the City or Municipal Health Officer of the Place of Origin).

For residential owners who are non-residents:

  • Land Title;
  • Building Permit;
  • Government Issued ID Indicating Residence;
  • Tax Declaration with 2019 Tax Clearance; and
  • Health Certificate Indicating Not a PUI or PUM (Issued by the City or Municipal Health Officer of the Place of Origin).

For land and condo unit owners who are non-residents:

  • Land Title/Condo Certification of Title for condo unit owners; 
  • Government Issued ID Indicating Residence;
  • Tax Declaration with 2019 Tax Clearance; and
  • Health Certificate Indicating Not a PUI or PUM (Issued by the City or Municipal Health Officer of the Place of Origin).

For visitors who have official business in Samal:

  • Travel Order Indicating Address in Samal; and
  • Company ID with Government Issued ID Indicating Residence

For Samaleños who are non-residents:

  • Government Issued ID Indicating Residence; and
  • Proof of being a Samaleño (Birth Certificate or Any Valid Documents.

For renters:

  • Government Issued ID Indicating Residence; and
  • Contract of Lease or Rent.

For students studying in Samal City, you must apply for a Student Pass ID. You may register for a student pass here: https://itracsamal.com/student_passes and upload a digital or scanned copy of your school identification card and your approved enrollment form.

For tourists or non-essential travelers, as of August 3, 2021, only guests/visitors coming from or within Region XI shall be accommodated in resorts and other accommodation establishments located in the Island Garden City of Samal.

How do I get a Samal QR Code?

To get a Samal QR Code, an individual must go to https://itracsamal.com/qr_codes and click the "REGISTER FOR A QR CODE ID" button. Once done, choose your type of registration (there are two options: Samal resident or non-resident), then fill out the form and upload the necessary documents required to get your Samal QR Code.

You may also follow the easy and simple steps below:



For desktop devices, open your web browser and then type itracsamal.com/qr_codes on the address bar to start your Samal QR Code registration. For Android users, it is advisable to use the Chrome browser, and for iPhone users, you may use the Safari browser.


2. Choose your type of registration and entry type


Once you're able to proceed with the ITRAC SAMAL QR code registration portal, you must click on the "REGISTER FOR A QR CODE ID" button to proceed. You will be redirected to a registration form afterward. Once you click it, you must choose your type of registration as mentioned here.


If you are a resident of Samal, choose SAMAL Resident, and if you are an outsider, choose Non-resident for your type of registration. After selecting your type of registration, you must select an entry typeFor residents, there are three options: (1) working in Samal, (2) working outside Samal, and (3) being Self-employed. 

For non-residents, there are nine options: (1) working in Samal, (2) business owner, (3) resort owner, (4) residential owner, (5) landowner, (6) condo unit owner, (7) visitors, (8) Samaleño, and (9) renters.

3. Fill out the Samal QR Code registration form


After you're done choosing the type of registration for your Samal QR Code, you may now fill out the form and provide the following details: (1) complete name, (2) gender, (3) date of birth, (4) email address, (5) mobile number, and (6) complete address.

You may fill out additional information based on your entry type, such as company name for workers and business name for business owners.

4. Upload the required documents



After filling out the registration form, you must upload a digital copy of the required documents. This depends on your entry type. You may check here for the documents needed upon getting a Samal QR Code ID. If you are a resident of Samal, you only need to provide a copy of your employment certificate and barangay certificate.

5. Click on the "Submit Application" button


Finally, click on the "SUBMIT APPLICATION" button once you fill out the registration form and upload the required documents. You will receive a reference number after you submit your Samal QR Code application. The reference number will be used to track your application status.



Once the verification is done, you will receive that your application for Samal QR Code ID is successful on your registered email. It usually takes a day for your application to be verified. You may check your application status at https://itracsamal.com/qr_codes by entering your reference number.


Why do we need to use Samal QR Code?

Suppose you are a resident or an individual staying in Samal. In that case, you need to have a Samal QR Code since it is required by the local government unit (LGU) per Executive Order No. 304 Series of 2021, particularly in the sections below:
 
"Checkpoints in all entry/exit points will continue to operate for the scanning of QR Codes of all travelers going in and out of the city. Individuals without QR Code shall not be allowed entry in the island.."

"All private and public establishments are required to adopt and use QR Code scanning in their respective places in accordance with the city's policies and guidelines on the use of QR Codes.."
