Steps and Requirements in Availing Social Services From OVP Satellite Offices

OVP satellite offices have started to provide social services such as medical and burial assistance. Learn the steps and requirements for availing of said services.

The Office of the Vice President's (OVP) satellite offices has begun to offer social services programs such as medical and burial assistance. Vice President Sara Duterte previously established satellite offices in several regions across the country to provide immediate services to the public.

Medical and burial assistance is available through identified partner hospitals or direct walk-in applications at the satellite offices.

The OVP currently has satellite offices in Dagupan (Region 1), Cebu (Region VII), Tacloban (Region VIII), Zamboanga (Region IX), Davao (Region XI), and Tandag in Surigao del Sur (Region XIII).

What are the steps and requirements to avail of services of the OVP satellite offices?

To avail of the services of OVP, applicants must meet the following requirements:

1. Complete the requirements needed for the application.

2. Fill out the application form. (You can get an application form in the satellite office or through this link: https://www.ovp.gov.ph/programs/medical-assistance)

3. Submit the complete requirements and form via walk-in to the satellite office processor/staff.

4. Wait for the processing of the application.

5. The processor/ staff will inform the applicant of the date of issuance of assistance that will be provided personally or via email, whichever is accessible.

What are the requirements for Medical Assistance?

1. Medical Assistance Application Form - Complete details must be dated and signed by the patient or his or her Authorized Representative. The form is available at OVP or on the OVP website (https://www.ovp.gov.ph/programs/medical-assistance).

2. Medical or Clinical Abstract/Medical Certificate - It must be signed, contain a Doctor's License Number, and be valid for 6 months.

3. Social Case Study Report or Barangay Certificate of Indigency

  • Social Case Study Report - Dated and signed by a Licensed Social Worker no later than one (1) year before applying for assistance. Can be addressed to the OVP or other government agencies.
  • Barangay Certificate of Indigency - Names of the patient and the Authorized Representative, as well as their relationship to one another (6 months validity).

4. Valid Identification Card of the patient and of the applicant - Front and back must be photocopied Bring your original to the interview.

Additional Requirements for Case Type


Medicines

  • Prescription - with physician's signature, license number, and expiration date (6 months validity)
  • Updated Quotation - a prescription drug's price quotation or price list. It is available at any pharmacy.
  • Treatment Protocol - signed by the physician and accompanied by a license number obtained from the medical record Validity period: 6 months

Hospital Bill

  • Endorsement Letter - signed and addressed to the OVP There are phone numbers for the hospital and the Medical Social Worker. For hospitals that are not OVP partners, bank information must be provided (Account Name, Account Number, Bank branch, and type of account.)

If the hospital refuses to accept the Guarantee Letter or provides an endorsement letter, a Direct Fund Transfer will be made to the patient or representative named in the Social Case Study Report.

  • Authorization Letter - Will be used if funds are not available in the patient's bank account.
  • Promissory Note (when the patient is discharged or discharged from the hospital) - signed by the hospital, patient, or Authorized Representative, including the accounting or credit collection officer's signature.

Chemotherapy, Radiation Therapy, Brachytherapy 

  • Treatment Protocol - signed by the physician and with a license number found on the medical record. (6 months validity)

Operation/Surgery/Transplant

  • Updated Quotation - the price of the operation/surgery as stated in the medical record. Signed by a physician and bearing his or her license number.

Implant (General)

  • Updated Quotation - a price quotation or price list for the medication mentioned in the medical record. It is available from any supplier.

Laboratory/Diagnostic Procedure

  • Request Letter - signed and bearing the Doctor's license number
  • Updated Quotation - a price quotation or a price list for the laboratory/diagnostic procedure specified in the medical record. It is available from the hospital or clinic where the procedure will be performed.

What are the Requirements for Burial Assistance?

1. Application Form - The Authorized Representative must sign and date the application form. A copy of the form can be obtained from OVP or from the OVP website (https://www.ovp.gov.ph/programs/burial-assistance).

2. Death Certificate - Must be registered with the local civil registrar.

3. Social Case Study Report or Barangay Certificate of Indigency

  • Social Case Study Report - Dated and signed by a Licensed Social Worker no later than one (1) year before applying for assistance. Can be addressed to the OVP or other government agencies.
  • Barangay Certificate of Indigency - Names of the patient and the Authorized Representative, as well as their relationship to one another (6 months validity).
