How to Get Your VaxCertPH Certificate and QR Code

The VaxCertPH contains an embedded QR Code that authorities can scan to validate the legitimacy of a vaccination certificate. Learn how to obtain a VaxCertPH QR Code here.

(UPDATED 03/01/2022) If you have already received all of your vaccinations against the coronavirus disease and wondering how to obtain a vaccination certificate or VaxCertPh and its corresponding QR Code, you have come to the right place.

VaxCertPH is the Philippine response to the need for a national vaccination certification that can be used as an entry requirement when traveling abroad or even within the country. VaxCertPh was made available for Overseas Filipino Workers (OFWs) and international travelers during its soft launch.

Now, fully vaccinated individuals in the Philippines can access the VaxCertPH, which provides a self-generated vaccine certificate with a QR-code that they can use when traveling. It is accessible at https://vaxcert.doh.gov.ph/.

What is VaxCertPH QR Code?

Once an individual has completed the process and obtained their certifications, the VaxCertPH QR Code is a generated matrix barcode image embedded in the vaccination certificate. The certificate's quick response (QR) code can be scanned by entities wishing to verify the document's authenticity.

Authorities can verify its authenticity by visiting the VaxCertPH website at https://vaxcert.doh.gov.ph/ and clicking on the "scan" button on the homepage.

What are the requirements to get a VaxCertPH QR Code?

Since the vaccination certificate contains a VaxCertPH QR Code, an individual must first register and obtain certification. They must be fully vaccinated and have received any of their doses 48 hours ago or more, as it will take time for the LGU where they received their vaccination to upload their information in the VaxCertPH portal.

The following requirements apply:

  • Must be completely vaccinated; and
  • possess a vaccination certificate (details about vaccination must be entered in the portal).

Where to download VaxCertPH QR Code?

Individuals must go to https://vaxcert.doh.gov.ph/ and follow the on-screen instructions to obtain or download a VaxCertPH. They must fill out all of the required details to download a vaccination certification with a VaxCertPH QR Code.


How to get a VaxCertPH QR Code?

A fully vaccinated individual must obtain a vaccination certificate from the VaxCertPH portal to obtain a VaxCertPH QR Code, as it is embedded in it. To do so, navigate to https://vaxcert.doh.gov.ph/ and enter the required information. Once your record is located, you will automatically receive your certification.

Download a PDF version of your vaccination certificate containing your VaxCertPH QR Code. Additionally, you may follow the steps below:

1. Go to https://vaxcert.doh.gov.ph/ and click the "Continue" button


To start the registration process and get your QR Code, you must generate a VaxCertPH through vaxcert.doh.gov.ph. You must click on the "Continue" button on the homepage to proceed.

2.  Review the Data Policy before proceeding with registration


You must check the box to confirm that you have read and understood the data privacy statement on the following page. To proceed with the next step, you must also ensure that you are fully vaccinated and have a vaccination card. Click on the "Continue" button to move forward.

3. Answer the required information and enter your vaccination details


You will then be asked about your vaccination status if you received the dose 48 hours ago; just answer yes or no. If you answered "yes," you will be required to fill out your personal information and your vaccination details, including the date and place of your doses.

4. Review the information and enter the verification code


If you received the dose within the last 48 hours, you will be asked by the system if you are vaccinated. Simply answer yes or no. If you answered "Yes," you will be asked to fill out your personal information as well as the details of your vaccinations, including the date and location of your first and second doses.

5. Generate and download your vaccination certificate to get the QR Code


After reviewing and verifying the information you provided, you will be able to obtain your VaxCertPH by clicking on the "Generate Vaccination Certificate" button. 

You will then be redirected to the next page, where you can download a "PDF" file of the certificate, which includes a QR Code that authorities can scan to verify its authenticity.

When and where can I use the VaxCertPH QR Code?

When authorities request it, you can use the QR Code in conjunction with the vaccination certificate. If this occurs, simply present the VaxCertPH, along with any necessary identification documents. They will then scan the QR Code to ensure that your vaccination certificate is authentic and issued by the Philippine Department of Health (DOH).

Is the vaccination card the same as VaxCertPH?

The vaccination card issued by the local government unit where you get your doses differs from the VaxCertPH in that it includes a system-generated QR Code. Meanwhile, the vaccination card contains information about your immunization.

In any case, both documents are important because they demonstrate that you have been fully vaccinated.


Why am I unable to obtain my VaxCertPH?

According to the Department of Health (DOH), there are three possible reasons why your VaxCertPH cannot be generated by the portal:

  • The data you provided on the request form is incorrect. You must double-check the information on your vaccination card.
  • You have not received two doses of vaccination in the Philippines, or your vaccination is incomplete.
  • It's been less than 48 hours since your last dose. You must wait for your record to be uploaded into their database for more than 48 hours.

If none of the above reasons apply, your vaccination record details may have been entered incorrectly or not uploaded. Simply use the website to request an update to your record.

What steps must I take to update my record on the VaxCertPH portal?

If your vaccination information is incorrectly encoded or your vaccination certificate is still unavailable even though you have received your final dose for more than 48 hours. You can do the following:

  1. To begin the update process, click "Update record."
  2. You must upload a photo of your vaccination card, a photo of your government ID, your email address, and your phone number.
  3. The information will be sent to the LGU so that it can be validated.
  4. You will be notified via email or phone number when you can re-request a digital vaccination certificate.

What is the procedure for scanning a VaxCertPH QR code?


To scan a VaxCertPH QR Code, navigate the VaxCert portal and select the "Scan QR Code" option. You can also do the following:

  1. Open https://vaxcert.doh.gov.ph on your smartphone, laptop, or computer with a webcam.
  2. Select "Scan QR code." Make sure to check the box next to "Allow the website to access my camera."
  3. Check that the QR code can be seen on the camera.
  4. The red line should be placed in the center of the QR code.
  5. Make sure the area is well-lit, and the photo is clear.
  6. After scanning, you will be redirected to a page that confirms the authenticity of the VaxCertPH.

Note: The entire QR code should cover 70-80% of the screen. Keep the QR code in front of the camera at all times. While scanning, try not to move your hand too much.
